Chronic disease clinics

If you are suffering from one of the diseases below, or have done in the past you will be called in for a yearly review or asked to have repeat blood tests.

If you have a chronic disease, we would like to review you on a regular basis in order to ensure that the condition is kept under control. We would normally ask you to come to an appointment every six months/once a year; the appointment is either booked with the Nurse or the GP depending on the nature of illness and the level of the professional care required.

The doctor and nurse would check for different things during their assessments, and may ask you to attend more frequently, depending on your case.

When your appointment is due, we would normally invite you to make an appointment to see the appropriate member of staff.
